Please fill out the enrolment package, registration
questionnaire, and read the parent handbook. You will also need to fill out the immunization form and will need an up to date copy of your child immunization record.

Once you have your forms filled out please scan them and email them back to mapletonpreschoolBOD@gmail.com.
You will be contacted by our registrar with your following steps.
Please fill out this package if your child is attending for a second year.
